Main Article Content Abstract Introduction Rheumatoid arthritis RA is an autoimmune rheumatic disease which often found in daily practice and requires certain considerations in recognizing clinical appearance also managing the disease as it often causes permanent joint damage, disability, even premature death. This recommendation is expected to become the latest reference for diagnosis and management of RA in Indonesia. Methods The steering committee was formed by the Indonesian Rheumatology Association IRA to formulate key questions; conduct literature search, selection, and review; then formulate recommendation statements for diagnosis, therapy, and monitoring of RA. Furthermore, the steering committee determined the level of evidence and grades of the recommendations. After that, the level of agreement LOA was determined for each item by panelists including rheumatology consultants who have been appointed by IRA to represent Indonesia regions. Results The steering committee established 30 recommendations including diagnosis, the role of laboratory and radiology tests, general treatment, the use of glucocorticoids, sDMARD, bDMARD, and tsDMARD. This recommendation also discusses guidelines on monotherapy, combination therapy, treatment strategies treat-to-target, tapering, and continuous clinical remission. Treatment on co-morbidities and complications are also included in brief. Conclusion IRA recommendations regarding the diagnosis and management of RA was made by considering various aspects such as the availability of drugs and supporting facilities, socioeconomic and cultural conditions in Indonesia, as well as the latest research that can be applied to Indonesian population. Bagus Putu Putra Suryana, SpPD-KR mengatakan perubahan gaya hidup saat pandemi COVID-19 turut mempengaruhi risiko osteoporosis. "Pandemi ini, jelas dilaporkan bahwa kita mengalami keterbatasan aktivitas, lalu pola makan juga berubah, dan perhatian banyak fokus kepada pandemi. Itu menyebabkan risiko osteoporosis karena adanya perubahan tersebut," kata dr. Bagus dalam jumpa pers daring, dikutip pada Rabu. Saat disinggung mengenai apakah kurang aktif bergerak, terutama di masa pandemi di mana mobilitas dibatasi, menjadi faktor utama dalam kenaikan risiko tersebut, dr. Bagus mengatakan, memang penting bagi manusia terlepas dari usianya untuk aktif menggerakkan tubuh demi meminimalisir kemungkinan tersebut. "Mobilitas terhambat, tapi usahakan kita untuk bergerak di rumah atau di dalam ruangan. Olahraga teratur tidak hanya memperkuat tulang, mencegah kemungkinan osteoporosis, tapi juga memperbaiki keseimbangan tubuh," ujar dokter yang menamatkan pendidikannya di Universitas Brawijaya, Malang tersebut. Penting bagi masyarakat terutama generasi muda untuk memahami bahwa osteoporosis dan penyakit tulang lainnya bisa dicegah sedari dini dengan gaya hidup sehat. "Osteoporosis bisa dicegah jika kita memulai gaya hidup sehat sejak dini. Mulai dari masa kanak-kanak, dimana kita harus mencapai massa tulang puncak yang tinggi. Kalau nutrisi tidak bagus, dan kita jarang bergerak, kebiasaan itu perlu diubah. Biasakan untuk hidup sehat dan asup diri dengan nutrisi yang baik," papar dia. dr. Bagus melanjutkan, semua orang memiliki takaran dan cara olahraga yang tepat dan teratur. "Rutin latihan fisik dengan intensitas sedang sampai berat selama 30-60 menit dalam 3-5 kali dalam seminggu, untuk mengurangi risiko osteoporosis terutama di bagian yang paling mudah keropos yaitu pergelangan tangan, pangkal paha, dan yang di tulang belakang bagian bawah," jelasnya. "Selain olahraga, osteoporosis dapat dicegah dengan mengonsumsi kalsium yang cukup yaitu mg per hari mg/hari untuk lansia, Vitamin D 600 IU, protein, kalium, kolagen dan mineral," ujarnya menambahkan. "Patients with fractures and osteoporosis have increased after this two-year pandemic," he noted at a virtual seminar entitled "Let's Improve Bone Health, Prevent Osteoporosis" here on Thursday. The increase occurred because, during the pandemic, people's mobility reduced, and people tended to eat without control and did not take care of their bone health. For that reason, he appealed to the public to take good care of their bone health from an early age by consuming milk and Vitamin D. Foods such as tofu, tempeh, nuts, vegetables, and fruits can be consumed from a young age to strengthen bones. In addition, drinking milk and sunbathing in the morning are also essential to improve bone health. According to him, by taking care of their bone health from a young age, people can achieve maximal bone growth and a greater peak bone mass. He said that 30 percent of women aged above 50 have osteoporosis. Meanwhile, 20 percent of men aged above 50 have the condition.
